How to Obtain a DMT Free or Reduced-Fare Identification Card

To obtain a DMT identification card, visit the DMT office at 101 N. Jackson, Danville, Monday through Friday from 8AM until 4PM. Four types of identification are available to riders, detailed below.

Disability

  • Authorizes the rider to pay reduced fare to ride DMT buses.
  • Complete the blue “Request for Reduced-Fare Certification” form, available at the DMT office or in PDF format.
  • Disability must be verified on the form by a physician, rehabilitation professional or healthcare provider. A Medicare card is accepted in lieu of verification by a health care provider.
  • Bring the completed form in person to the DMT office.
  • If using your Medicare card to verify disability, bring it with you when you come to the office. Also bring state photo identification or a driver’s license OR another document with your name and birthdate and second form of identification with name.

Medicare

  • If you have a Medicare card you are eligible to pay reduced fare on all DMT services (except ADA paratransit). You will need a DMT ID to pay the reduced fare. To obtain one, bring your Medicare card to the office. Also bring state photo identification or a driver’s license OR another document with your name and birthdate and second form of identification with name.

Benefit Access Program

(Formerly known as Circuit Breaker)

  • Authorizes riders qualifying for the state Benefit Access Program due to disability to ride DMT fixed route buses for free (does not apply to paratransit buses).
  • You must have completed enrollment in the state Benefit Access Program. See www.cbrx.il.gov. Your enrollment will be verified by DMT staff before we can issue you a Benefit Access Program identification.
  • Complete the green “Request for Free Ride Certification” form, available at the DMT office.
  • Bring state identification or a driver’s license OR another document with your name and birthdate and second form of identification with name.

Senior 65+

  • Authorizes riders age 65 and over to ride DMT fixed route buses for free (does not apply to paratransit buses).
  • Complete the pink “Request for Free Ride Program” form, available at the DMT office or in PDF format.
  • Bring state identification or a driver’s license OR another document with your name and birthdate and second form of identification with name.

Paratransit

  • Certifies riders for use of DMT paratransit services (which are operated by CRIS Rural Mass Transit District under contract with DMT). This service is reserved only for riders who have disabilities that prevent them from using fixed route services.
  • Complete the grey “ADA Paratransit Request for Certification” form, available at the DMT office or in PDF format.
  • Disability must be verified on the form by a healthcare provider. The health care professional must be very specific concerning how the disability prevents the rider from accessing fixed route buses.
  • Return the completed form to DMT for eligibility determination, which may take 7-10 weekdays.
  • If you are approved you’ll need to come to the DMT office to have a photo identification card made before you can begin using the service.
